Thank you for your commitment to serve at-risk children through the ministry of Kids Alive. We are excited about your upcoming trip and look forward to working with you during your preparation for the field. Please use the attached checklist to see which forms must be printed from our website and turned in 30 days prior to your departure. Please also use the additional resources at the bottom of the page to help in the organization of your team.

You should follow the medical advice of your healthcare provider, particularly if you are pregnant or planning to become pregnant, when considering travel to a Zika-affected country (including the Dominican Republic, Haiti, Peru, and Guatemala). PLEASE NOTE: All forms should be submitted to your Team Leader at least one month prior to your team’s departure! Medical Information forms and Parental Agreement for Minor forms will be kept by the Team Leader during the trip in case of emergency. All other forms will be collected by the Team Leader, and then submitted together by the Team Leader to Kids Alive International.
Support Raising Processes
We understand and appreciate that service team members are often times raising funds to participate in these trips. In order to do so, we’d like to give you a few recommended support raising books to help in the process.

Check the insurance policy that you are currently covered under. If you need accident and health coverage outside the USA, or wish to purchase additional travel insurance, you may enroll in a temporary insurance package specifically for short-term service. We are partnering with MissionTripInsurance.com to offer two different plans to our teams: www.missiontripinsurance.com/kids-alive/
If you decide to purchase temporary insurance through another provider, be sure that it will cover you overseas.
It is required that every Kids Alive Short-term Missionary and Service Team Participant have health and accident insurance.
Passports are required to enter all of our locations around the world. Please make sure that you have a passport, and/or apply for one in a timely manner prior to your trip.

Kids Alive encourages team members to use the CDC’s website and collaboration with their physician’s recommendations in order to determine what immunizations are necessary for upcoming travel. Please note that many immunizations may be available at your local health department for a reduced rate.
